Barbra S. London, MSW, LCSW, LNHA

President

Ms. London has over 30 years of experience. She received her master’s degree in social work, and is a licensed clinical social worker (LCSW) as well as a licensed nursing home administrator (LNHA) in the state of New Jersey.

Diane M. McCullom, BSN, RN

Vice President, Clinical Operations

Ms. McCullom received her bachelor of science in nursing degree from Russell Sage College in 1981, and has worked in home health and hospice management for over 20 years.  Ms. McCullom joined the Freedom Team in 2010.

Candy Blau, MA

Regional Director of Sales and Marketing

Ms. Blau received her master’s degree in gerontology in 1976 and has worked for over 35 years assisting older adults and their families. Ms. Blau joined the Freedom team in 2008.

Dawn Perdon, BA

Senior Eldercare Advisor

Ms. Perdon received an associate’s degree in gerontology in 1995 and a bachelor’s degree in sociology in 1998. She also holds a certificate in gerontology. Ms. Perdon has worked in home health care and assisted living for 15 years, and joined the Freedom team in 2009.

Rachel Cohen

Senior Director of Client Services

Rachel Cohen joined Freedom Home Healthcare in August 2015 after nearly a decade of serving as the Director of JVS Caregiving Companions and Home Maintenance Solutions. As an MSW from the University of Chicago, Rachel brings her skills and passion with the hope of ensuring the care and support older adults and their families need throughout the aging process. Rachel understands that it “takes a village” and feels privileged to work with the Freedom team, including our dedicated and committed caregivers, in her role of Senior Director of Client Services.

Sharon Mish, RN

Director of Clinical Services

Mrs. Mish has been an outstanding nurse for Freedom since she first joined the team in 2013.  She has a wealth of experience in long-term, assisted living and home care nursing.  Sharon has first-hand experience working in specialties that include:  Quality control, clinical compliance, management and staff development.  She has a certification in, AL, dementia/Alzheimer’s care, medication aide and certified home health aide education. Sharon has devoted her entire nursing career to caring for the elderly.

Elina Polyakov, MSW, LSW

Director of Social Work & Care Management

Ms. Polyakov is a master’s prepared social worker who helps clients and their families with the transition from wellness and independence to the need for various levels of assistance. She serves as a liaison to caregivers in the home, and provides them with coaching, counseling and techniques necessary to succeed  in various settings  and cultures.
