Geriatric Care Management
As a loved one ages, and their physical and/or cognitive abilities decline, family members may not know where to turn for assistance in understanding the aging process and exploring care options that are appropriate for their loved one. Freedom’s Geriatric Care Managers can assist seniors and their families with the process of identifying and implementing optimal solutions for short and long-term needs. Our Geriatric Care Managers also focus on the development and coordination of a customized plan of care that is designed to meet each client’s needs.
Complimentary home visits can be scheduled to address a client’s changing care needs or to discuss concerns that arise once our home care services are in place. Initial and ongoing communication is vital. This ensures that all parties clearly understand the client’s needs and expectations and enables our Freedom team to provide the high quality care and extraordinary service on which we pride ourselves.
Major life changes can be difficult regardless of one’s age. For seniors, change of any kind can be traumatic. When a crisis arises, when there is a change in a client’s mental or physical status or when a comforting, friendly face is needed, our Geriatric Care Managers will be there to support our clients and their family members.
